Security preparation, training, and monitoring were found severely lacking in the prison.
The report recommended strengthening jail doors, walls, control points and advised installing a 24-hour modern CCTV system along with providing crisis response training to officers and staff.
Further recommendations included allocating emergency zones for prisoners and staff, placing motion sensors and alarm systems at sensitive points, and establishing a separate armory.
Anti-riot equipment and drones should also be secured for emergency use.
