To apply, you must provide both parents’ original CNICs, the child’s computerized birth certificate, and legal adoption proof if the child is adopted officially.
Children under 3 years do not need to present at NADRA. Children aged 3-10 need a photo. Ages 10–18 need photo, fingerprint scan, and eye scan.
Adoptive parents must get the form attested by a gazetted officer or elected representative before submission, along with adoption-related identity documents.
How to Apply
Visit the nearest NADRA Registration Centre. Collect a token from the reception desk and wait for your turn to appear on the digital screen.
Once called, one parent’s biometric verification is completed to confirm identity before the child’s data is entered in the NADRA system by the operator.
The officer enters the child’s data, captures a photograph and fingerprints, and prints the form. Parents must review all details and ask for corrections.
