Parents and legal guardians applying for their child’s B-Form, also known as a Child Registration Certificate (CRC), for the first time should ensure they have all the required documents before visiting a NADRA Registration Centre.

The National Database and Registration Authority (NADRA) has outlined the documents and requirements that parents and guardians should follow when applying for a child’s B-Form.
Documents Required for B-Form
Parents or legal guardians should keep the parent’s or guardian’s CNIC number ready along with a verified computerised birth certificate issued by the relevant Union Council, Municipal Committee or Cantonment Board.
For children born abroad, additional documents may be required, including a detailed foreign birth certificate, S-1 form issued by a Pakistani embassy or mission, citizenship certificate or naturalisation certificate, depending on the child’s circumstances.
Children Over Three Must Visit NADRA Office
NADRA states that children above the age of three must be brought to the NADRA Registration Centre for biometric and identification requirements.
These include capturing the child’s fingerprints, photograph and iris scan.
Parents should therefore ensure that eligible children accompany them when visiting the registration centre.
Requirements for Legal Guardians
If the application is being submitted by a legal guardian, the guardian must provide a guardianship certificate issued by a competent court.
This document is required to establish the guardian’s legal authority to apply for the child’s registration.
Photograph Requirement for Passport
NADRA has also clarified that if a passport is to be obtained for the child after the B-Form is issued, the child’s photograph must be captured as part of the B-Form registration process.
Parents are advised to provide complete and accurate documentation to avoid delays or inconvenience during the registration process.
